Step Two - Patient Registration
Follow these steps:
Go to the patient portal (https://32344-1.portal.athenahealth.com/) to click on and complete these sections. Having trouble? Please call us at 414-999-3815.
My Profile
- Find in the main menu, or by clicking on the arrow at top right.
- Enter home address, phone number, email address, emergency contact.
- Say “yes” to all text and/or phone alerts.
Billing/Payments
- Enter your insurance information: Go to "manage insurance," then "add insurance" to upload pictures of both sides of each card.
- Enter a credit/debit/HSA card on file: Go to "payment methods" and click/tap on the "+".
- Workers' compensation / alternate coverage / no insurance? Please contact us.
My Health
- Go to “Forms and Documents” to complete: Notice of Privacy Practices, Financial Policy, Patient Rights and Responsibilities.
- Enter any additional health history information.
