Thank you for entrusting us with the care of your patients!
- We DO take referrals from all sources, though urge all patients with chronic pain to have a primary care provider. Additionally, some insurances require primary care referral.
- Please fax referrals to 833-428-4315, and include:
- Who is referring, pain location, patient contact information, insurance if possible
- If workers' compensation, please include carrier and claim number if available
- We will reach out directly to all patients, but please also encourage them to visit www.kineticspineandpain.com for fast and easy online self-registration and scheduling.
- Please note, we are not able to provide long-term opioid management.
- Our philosophy is to help minimize reliance on medication through the use of physical medicine & rehabilitation, interventional, mind-body, and self-management strategies.
- For patients interested in participating in opioid reduction, we may take over prescribing while this process is ongoing. However, if no medication changes have been made in six months, the patient will be expected to transition stable refills back to their primary care provider. No opioids will be prescribed at the first visit, and such decisions will be made only after evaluation and open discussion with the patient.
